Register New Declaration

Specification

1. Check existence of $doctor_id and is_active = true.
   1. If doesn't exist - return error
2. Check existence of $msp_id and is_active = true.
   1. If doesn't exist - return error
3. Create new declaration record
4. Default Declaration status = "PENDING_SIGNATURE"

TODO: deactivate old declaration.
